Early Childhood Screening is a FREE developmental screening required for all preschool-age children by the State of Minnesota prior to entry into kindergarten and some early childhood programs. The best age to screen is 3 ½ but children can be screened at age 3-5. Screening benefits children and families.

At your child’s screening appointment, a teacher or nurse will check your child’s:

• Vision, hearing, height, and weight
• Social and emotional progress and overall development
• Immunizations and health history

Early childhood developmental screening helps:

• Parents confirm their child is on track with development
• Connect you with early childhood programs and services
• Answer your parenting questions

Schedule an Appointment

Please follow all steps in order.

1. Complete online enrollment using this link or the enroll section of our school district website. 

   • Select "Crystal Lake Education Center Programs"
   • Submit your contact information to receive an email with a unique link to your online forms. The email will come from messenger@isd194.org with the subject line “Online Enrollment Application for Crystal Lake Education Center.”
   • When you are ready, click the unique link provided in the email to begin the enrollment process
     • Please allow for approximately 20 minutes to complete the paperwork. 
     • Under the student tab you will choose Early Childhood Screening as your enrollment option. *If you are also enrolling in Small Wonders Preschool and/or ECFE you may choose to enroll in multiple programs with one application!

2. Complete the Parent Questionnaire 

   • You will receive two emails from messenger@isd194.org. The first email confirms your forms have been submitted. 
   • The second email will generally come one business day after your forms have been submitted. This email will contain a link and instructions to a parent questionnaire called the ASQ-SE2. This lets us hear from you, the parent, about your valuable perspective on the social and emotional development of your child. 
     • To fill out the parent questionnaire immediately, go to and choose the third option: “I am completing the ASQ:SE-2™ questionnaire.”

3. Once we have all your forms and surveys, we will send you a link to choose a time to come to Crystal Lake Education Center for your child's screening appointment.  

Screening 

In-person appointments are for parent and child together, and take approximately 45 minutes to 1 hour. Screenings are held at Crystal Lake Education Center, 16250 Ipava Ave, Lakeville. For the health and safety of our staff, students, and those being screened we ask that you follow the safety guidelines: 

• One adult is permitted to accompany the child to the screening appointment and will be asked to wait outside the room during the developmental screening process.
• No other siblings or children can be admitted to the building for the screening.  
• Prior to screening the adult and child will be asked to wash hands before entry to the screening room. 

After the screening is completed, you will receive a copy of the summary sheet and the form will be sent to your child’s elementary school prior to their entry to kindergarten.
